Pathophysiology
Atherosclerotic plaques develop silently over decades via endothelial dysfunction, LDL infiltration, foam-cell formation, and a fibrous cap. When the cap thins, it ruptures exposing the lipid core; tissue factor triggers the coagulation cascade, glycoprotein IIb/IIIa receptors on activated platelets bind fibrinogen, and an occlusive thrombus forms. The downstream consequence is a supply-demand mismatch — coronary flow drops below myocardial oxygen demand, producing ischemic chest pain, regional wall-motion abnormality, and, if unrelieved, myocyte necrosis beginning subendocardially and spreading transmurally.
