In Saudi Arabia, the specialty is regulated by the Saudi Commission for Health Specialties (SCFHS) and practised within the Ministry of Health (MOH) primary healthcare centres (PHCs) that act as gatekeepers for the referral pyramid. The SCFHS residency is 4 years (R1–R4) including the Arab Board / Saudi Board final examination. The six core competencies from WONCA are: primary care management, person-centred care, specific problem-solving skills, comprehensive approach, community orientation, and holistic modelling. Definition and Scope

Family medicine provides first-contact, comprehensive, continuing, coordinated, community-oriented, and family-centred care. Unlike organ-defined specialties, family physicians manage undifferentiated presentations — chest pain, fatigue, abdominal pain — before a diagnosis exists, and they often manage the patient across decades. The biopsychosocial model (Engel) replaces the purely biomedical view by integrating psychological, social, family, occupational, and cultural factors.

The SCFHS Training Pathway

The Saudi Board of Family Medicine is a 4-year residency (R1–R4) under SCFHS, with rotations in internal medicine, paediatrics, obstetrics, emergency, psychiatry, and electives, ending in the Saudi Board Final Examination (SBFM) — written (MCQ + EMQ) plus OSCE/structured oral. Holders are eligible for the Saudi Classification and Registration as a consultant family physician and the Arab Board dual pathway.

The WONCA Core Competencies

The 2005 WONCA framework defines six: primary care management (gatekeeping, resource use), person-centred care, specific problem-solving (early, undifferentiated), comprehensive approach, community orientation, and holistic modelling. The GP Board often asks candidates to apply one competency to a vignette.

Primary Care Procedures

Within PHCs the family physician performs: suturing and wound care, incision and drainage, joint and soft-tissue injections, Pap smear, basic fracture splinting, neonatal resuscitation, and cryotherapy. Procedural competence distinguishes the family physician from a triage nurse and is regularly tested as OSCE stations.

Preventive Services and Common Case Mix

Standard preventive packages include childhood immunization (MOH schedule), antenatal care visits, postnatal and well-baby checks, and adult screening per MOH/CHO guidelines. The most common Saudi PHC presentations are type 2 diabetes mellitus, hypertension, asthma, upper respiratory tract infections, vitamin D deficiency, low back pain, and depression/anxiety.

The GP Board commonly tests: (a) choosing the next step in an undifferentiated presentation; (b) screening intervals (e.g. HbA1c every 3 years > 45 y, mammography biennial 40–69); (c) referral vs manage in PHC thresholds; (d) biopsychosocial assessment of a frequent attender.

Gatekeeping and Referral Logic

The family physician is the entry point of the Saudi health system; ≥80% of encounters should be closed at PHC. Correct referral is a balance: under-referral misses red flags (cord compression, ACS mimics, paediatric sepsis); over-referral saturates hospitals. Practical criteria for referral: diagnostic uncertainty beyond PHC scope, failure of first-line therapy after an appropriate trial, need for specialised procedure or imaging not available at PHC, and red-flag symptoms (sudden severe headache, neurological deficit, haematemesis, postpartum haemorrhage, neonatal jaundice in first 24 h, suspected child abuse).

Cost-Effective, Evidence-Based Practice

EBM at the primary-care level means choosing tests with high likelihood ratio, applying number-needed-to-treat (NNT), and avoiding low-yield panels (e.g. annual ECG in low-risk adults). The Saudi PHC formulary restricts antibiotics and combination products; candidates should know rational prescribing of antibiotics for URTI (do not give for viral pharyngitis), stewardship in DM/HTN, and deprescribing in polypharmacy patients > 65 years.
